This role is perfect for you if:o You're passionate about health and social care.o You possess excellent communication and interpersonal skills.o You're a team player with a strong work ethic.o You're eager to learn and grow in your career.The Lambeth CHC Team within the South East London ICB is looking for a band 8A nurse with a difference to oversee the clinical work of our CHC assessment and case management team as well as assist in driving forward our local transformation programme aimed at improving the performance of the team.
The main purpose of this role is:o To provide expert clinical advice and support to the ICS and the CHC Commissioning Team in commissioning high-quality cost-effective NHS funded care for adults who have been assessed as eligible for NHS Continuing Healthcare.o To lead on Appeals and NHSE IRP panel presentationso To have oversight of all CHC assessments and reviews of individuals eligible for NHS Continuing Healthcare, where required; as well as reviewing individual care packages and one to one funded arrangements.o To work with the relevant community teams responsible for case managing CHC funded cases to support the delivery of quality commissioned care, linking and collaborating with other health and social care providers in primary, secondary, independent and voluntary care,o To promote communication, seamless care and support for patients and their carers.o To work with the Commissioning Manager - CHC lead to provide specialist advice and training for the local health and social care workforce.

We work in a hybrid way and time in the borough (Lambeth) is required as per the needs of the service.
