Job description
The Clinical Pharmacist will work as part of a MDT and pharmacy team comprising of other clinical pharmacists, pharmacy technicians and pharmacy administrators. The post holder must be passionate about delivering high quality patient care, ensuring patient and drug safety, and in providing holistic patient centred care. The clinical pharmacist will conduct structured medication reviews, LTC reviews, deprescribing where appropriate, manage care home patients and patients with frailty mediation reviews, support the wider practice team with drug related queries, support drug monitoring processes and drug safety. The clinical pharmacist will support the lead pharmacist, and with the rest of the pharmacy team, PCN CD and PCN manager in ensuring that targets around the PCN DES, IIF and practice QOF, drug safety and monitoring searches are met.

About us
The following are the core responsibilities of the PCN Clinical Pharmacist post. There may be occasions, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
- Clinical pharmacists will work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines
- They will be prescribers, or will be completing training to become prescribers, and will work with and alongside the general practice team.
- Clinical pharmacists will undertake medication reviews to proactively manage people with LTC, people in care homes, frailty, and people with learning disabilities or autism (through STOMP Stop Over Medication Programme).
- Clinical Pharmacists will provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the PCNs practice and to help in tackling inequalities.
- Clinical pharmacists will provide leadership on person centred medicines and quality improvement, whilst contributing to the QOF and both practice /PCN enhanced services.
- Through structured medication reviews, clinical pharmacists will support patients to take their medications to get the best from them, reduce waste and promote self-care.
- Clinical pharmacists will have a leadership role in supporting further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.
- The role has the potential to significantly improve quality of care and safety for patients.
- The Clinical Pharmacist will develop relationships and work closely with other pharmacy professionals across PCNs and the wider health care systems
- Clinical pharmacists will take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning difficulties), liaison with community pharmacists and anticoagulation.
- Take part in Clinical Audit and quality improvement to ensure prescribing is in line with local and national guidance, CQC requirements and up to date MHRA /CAS alerts.
- Clinical pharmacists as appropriate will support the Lead Clinical Pharmacist in mentoring providing training /support for the Pharmacy Technicians
- Support the wider clinical team with education and induction programmes for new clinical staff
- Support the Lead Clinical Pharmacist in ensuring appropriate representation at local ICB Medicine Management Team meetings and other PCN/practice meetings
