The Clinical Services Care Coordinator is primarily responsible for supporting the Clinical Services Team with regards to delivery of services offered at the practice. The focus of the team is to ensure high quality care for patients through a well-co-ordinated administrative system, which offer a holistic approach to improve patient outcomes and experience.

The role includes - providing administrative support to help deliver GMC and non GMS work including the Quality Outcome Framework (QOF), Enhanced Services, AQP Contracts and any other schemes as required.

Liaising with and supporting the Clinical Team and the practice staff as well as other contractors that are associated with the practice as well as third parties including contractors.

Helping to improve systems and processes including utilising software/technology to improve patient care and outcomes.

Provide administrative support to the Clinical Services Team with regard to GMS and Non GMS work including the Quality Outcome Framework (QOF), Enhanced Services, AQP Contracts and any other schemes as required

Liaising with and supporting the clinical team and the practice staff, other contractors that are associated with the practice as well third parties as necessary

Effective use of IT/software to assist in management of practice population including EMIS, Ardens and iPlato etc

Utilise EMIS searches and reports for chronic disease monitoring, other monitoring and clinical audits as required

Assist with mail merges and text messaging campaigns to support chronic disease management, vaccinations campaigns as well as other information which may include health promotion or patient feedback

Support Coding Team with filing documents and basic coding as required

To attend internal and external meetings as appropriate

To keep up to date with mandatory and role specific training as well as attend training courses and other events relating to the role.

Helping to solve day-to-day issues with IT, systems, workloads, etc as they arise

To keep up-to-date with the Policies and Procedures of the organisation

Undertake any other duties appropriate to the role.
