This role supports an already established team of dietitians and discharge coordinator who manage patients who are tube fed at homeThe clinical caseload will require a mix of face to face and virtual service provision.

Main duties of the job
Working within defined scope of practice and service guidelines, key aspects of this role will includeassessing the nutritional needs of non-complex adult patients with a range of medical conditions,devising programmes of dietetic care under dietetic supervision,monitoring and evaluating the effectiveness of dietary intervention and providing regular feedback to the supervising dietitian.supporting team with the discharge and ongoing administrative management of patients who are fed via a feeding tube in the communityliaising with dietitians, community staff, home enteral feeding company, ward staff dailyFull training will be provided but candidates must have a relevant qualification and must be able to demonstrate nutrition knowledge as well as excellent communication and organisational skillsOur service delivers care across 2 primary care hospitals and within community settings across the South Tees locality; thereforesuccessful candidate must have access to a vehicle for work purposes to meet the requirements of the roleProvisional interview date: 27th November 2025
