Make a tangible difference supporting people through colorectal cancer treatment and recovery as a Colorectal Cancer Support Worker with Hampshire Hospitals NHS Foundation Trust. Based at Basingstoke (RG24) with some travel to Winchester, you will be the key contact for patients on Open Access Follow‑Up, delivering Level 1 psychological support, Holistic Needs Assessments and telephone triage within agreed protocols. Day‑to‑day you will help patients and carers navigate care pathways, support health and wellbeing programmes, collaborate closely with Clinical Nurse Specialists and the wider multidisciplinary team, keep clear records and help people access the right community and specialist services.

This permanent, full‑time role is Agenda for Change Band 4 (salary £27,485–£30,162 per annum pro rata) and suits experienced HCAs, social care workers or carers with NVQ Level 3 (or equivalent) and at least two years’ healthcare experience who value person‑centred practice and good communication skills.
