Community Band 3 HCA role within Mid Cheshire Hospitals NHS Foundation Trust’s District Nursing Service (SMASH), based in Sandbach (CW11). This permanent, part-time post involves delivering nursing interventions in patients’ own homes – including wound care, phlebotomy, medication administration and risk assessments – and supporting people approaching the end of life. You will work with delegated clinical tasks from the Community Nurse team, often with minimal supervision, so sound clinical judgement, good time-management and the ability to travel across the Central Cheshire Integrated Care Partnership footprint are essential. The role is Agenda for Change Band 3 (salary £24,937–£26,598 pro rata), requires NVQ Level 3 in Health Care (or equivalent) and is subject to a Disclosure and Barring Service check; applicants who require skilled worker sponsorship will be considered.

This opportunity will suit compassionate, resilient carers — particularly women looking for meaningful community work that fits around life commitments — who enjoy building empathetic relationships and working as part of a multi-disciplinary team. You’ll receive ongoing training and support to develop community nursing skills such as phlebotomy and IT-based record keeping, and you’ll be expected to contribute to a patient-centred, rehabilitative approach. If you’re reliable, approachable and keen to provide dignified end-of-life care while working flexibly across local services, find this vacancy on jobsincare.com and click the “Apply Now” button to submit your application.
