The post holder will be responsible for a mix of duties including leadership responsibility for the Discharge to Assess Pathway1 service (reablement in people’s own home) and for the Enhanced Delirium Pathway. These services deliver reablement led assessments of long term health and social care needs, either following discharge from hospital, or in response to preventing an unnecessary admission. Elements of the service have recently transferred from Derby City Council to Derbyshire Community Health Services NHS Foundation Trust (DCHS) and the two organisations are continuing to work in partnership to develop an integrated service with the associated DCHS teams that support Discharge to Assess and Urgent Community Response (admission avoidance). As a member of the Leadership Team this role will play a key part in the transformation work, maximising the opportunities around integration and ensuring the delivery of efficient, high quality service. Although the post is advertised as a non-clinical role applicants will be accepted from a clinical background. The Service Lead will: Work alongside clinical and operational leaders to ensure effective co-ordination and management of services, budgets and agreed national/local performance objectives. • Provide management and oversight of the Pathway 1 Support Workers and for the administration staff and functions which support the delivery of the service including the programming team. • Lead and initiate on service wide project work which supports the development and continuous improvement of care processes and pathways. This includes the embedding of service innovations which increase the accessibility and effectiveness of the service. • Positive working relationships will be established with colleagues in other organisations and teams with whom their service interacts and with external bodies. • Ensure the smooth day-to-day operation of the services including overseeing the rostering of shifts and scheduling of clinic activity via SystmOne and e-Rostering systems. • Work collaboratively with other leaders to develop an open and robust communications system within the service. • Oversee the development and smooth running of efficient systems to enable activity reporting, risk register management, appraisal and sickness/absence management. • The post holder will look to support a multi-disciplinary and collaborative approach to care and be responsible for establishing and promoting the Trust’s values.
