As a Community Healthcare Assistant with HCRG Care Group based in Chertsey (KT16), you will support children, young people and their families across homes, schools, early years settings, clinics and community venues. Working under the supervision of qualified professionals in the Surrey Healthy Family Team, you will help deliver universal, targeted and specialist programmes such as the Healthy Child Programme, assist with assessments and progress monitoring, run group sessions and workshops, and contribute to care planning by adapting interventions to individual needs. The role involves accurate, contemporaneous record‑keeping, following safeguarding procedures, managing a caseload within organisational policies and lone‑working guidance, and using clinical reasoning to make referrals where needed. This permanent, full‑time post offers flexibility through office hubs and hybrid working, a starting salary of £26,712 to £29,494.50 (FTE), and requires a DBS check and access to a car insured for work purposes.
