Are you ready to create a meaningful impact in the lives of adults with learning disabilities in Croydon? Join our dedicated Community Learning Disabilities Team at the Croydon Health Services NHS Trust in a full-time permanent role as a Community Learning Disability Nurse. We’re looking for passionate individuals who can use their expertise to improve health outcomes for those with complex needs, including neurodevelopmental conditions and mental health challenges. You’ll work closely with a supportive multidisciplinary team, based in the heart of Croydon, alongside health professionals such as Psychologists, Speech & Language Therapists, and Dietitians.

In this rewarding role, you will conduct comprehensive specialist assessments and develop personalised Health Action Plans, ensuring that each individual's unique needs are met. Collaborate with other professionals to provide effective care for individuals with learning disabilities, including those exhibiting challenging behaviours.
