As a Community Nurse covering Totton (SO40 9LA) and Bitterne (SO18 6BT) you will manage a defined community caseload, delivering complex care and treatment to adults with long-term or complex needs. Working within an Integrated Care Team alongside therapists and specialist services, your role is to carry out holistic assessments, create and record care plans, coordinate referrals and support reablement, rehabilitation and health promotion. This Band 5 post is open to newly qualified and experienced NMC-registered nurses, with responsibilities requiring professional accountability, strong record-keeping and the ability to work with autonomy to prevent unnecessary hospital admissions and enable timely discharges.
