As a Community Physiotherapist for NHS Jobs in Lewisham (London SE13), you will lead, manage and deliver physiotherapy services across the community, reporting to the Head of Therapy. This permanent, full‑time post (salary £47,810–£54,710; job ref C9197-25-1084) is central to the discharge pathway and liaises closely with the Acute Trust, CCG/ICS and the London Borough of Lewisham. You will undertake holistic assessments and evidence‑based treatment for people with complex needs — including older adults, neurological and acutely ill patients — start treatment in the acute setting when required, and plan, evaluate and adapt caseloads to meet patient priorities. The role includes legal and professional accountability for your practice, safe use and training in equipment, completion of reports and discharge summaries, and participation in multidisciplinary meetings and care planning.
