Job description
Support adults across Hampshire as a Community Reablement Assistant (Level 2), delivering hands-on reablement care that restores independence and helps people stay safely at home. You’ll carry out initial home visits and lead reablement interventions — including intimate personal care, continence support and safe moving and handling — while assessing risks, setting achievable goals and updating care plans. The role includes supervising and coaching Level 1 Assistants and working closely with Team Leaders, Occupational Therapists, Sensory Officers and NHS colleagues to enable rapid hospital discharge and prevent unnecessary admissions. This practical, people-centred post suits experienced carers or those with health and social care backgrounds who bring patience, compassion, excellent communication and a commitment to dignity and respect.
This is a permanent, full-time role based in Hampshire with a salary of £27,780–£30,564 per year and access to the Local Government Pension Scheme, Health Assured employee support, generous annual leave, occupational sick pay and clear pathways for professional development. The service runs 07:00–22:00 every day, so you’ll be required to work evenings, weekends and bank holidays as part of your normal hours (enhancements apply). You’ll need MO365 skills (Word, Outlook, Excel), previous experience in a social or health care setting and the right to work in the UK (no sponsorship available).
