Job description
Work as a Community Support Worker in Bridgend County Borough Council’s Home Treatment Team, based at the Princess of Wales Hospital. This permanent, full‑time role (37 hours/week) offers a salary of £28,598–£29,540 and centres on providing recovery‑focused support to adults with severe and enduring mental ill health, often with co‑occurring substance misuse. You will carry out outcome‑focused assessments, deliver brief interventions, signpost to local services and work closely with a multi‑disciplinary team to prevent avoidable hospital admissions and support early discharge. The post sits within a strengths‑based model of practice and is line managed by the Senior Social Work Practitioner in the Home Treatment Team.
