Support community patients at risk of malnutrition as a Band 4 Dietetic Assistant Practitioner based at Ty Gwent, Llantarnam, Cwmbran (NP44). This fixed‑term post (until 31/12/26) offers a full‑time, hands‑on role working alongside registered community dietitians to carry a delegated caseload in community hospital settings. You’ll help deliver compassionate, practical nutrition care, undertake administrative tasks to support patient pathways, and use IT systems to maintain records. Prior experience in care or inpatient settings is helpful but not essential — training will be provided. The salary sits on Agenda for Change Band 4 (£27,898–£30,615 pa) and the post requires relevant nutrition qualifications (or equivalent NVQ/Level 3) and good communication skills; Welsh speakers are welcome.
