This is an exciting opportunity to be a part of the Integrated Discharge Team within WVT as a Band 4 Discharge Co-ordinatorWe are looking for motivated and compassionate staff to focus on the patient discharge planning process supporting the safe and effective discharge for patients.If you are interested in the role and believe you hold the qualities we require, then apply today!You need to be organised, able to prioritise workloads, use your own initiative and communicate clearly and concisely with a range of people. The role will involve working closely with ward staff, twice daily ward MDT meetings using the ethos of 'Why not Home, Why not Today' and communicating with patients, relatives and other community services to provide a proactive and continuous process of discharge planning, avoiding delays and thereby improving patient

Main duties of the job In this vital role, you will:Work in partnership with other members of the Integrated Discharge Team (IDT), Ward Multi-Disciplinary Team (MDT) and Community partners, to support decisions with regards to discharge planningRepresent the IDT at ward daily board roundsProvide effective & timely communication of discharge actionsBe expected to undertake a range of duties without direct supervision but will be required to report back on those delegated duties to the ward team and case managerLeasing with patients and families regarding dischargeWe offer an induction period upon joining the team to introduce you to all the community services in Health and Social care who support discharge planning. We also offer a range of training opportunities to support the development of the right candidate.
