This internal opportunity at Wye Valley NHS Trust in Hereford is for a Band 4 Flow & Discharge Coordinator working within the Integrated Discharge Team to drive safe, timely discharges across acute and community services. You will work closely with ward teams and community partners, represent the IDT at daily board rounds and twice-daily ward MDT meetings, and use the ‘Why not Home, Why not Today’ ethos to reduce delays. The role suits organised, compassionate carers who can prioritise, use their initiative and communicate clearly with patients, relatives and multidisciplinary colleagues. You’ll be expected to lead on delegated discharge actions, liaise sensitively with families, and contribute to continuous improvement in patient flow — a hands-on post that makes a direct difference to people returning home or into appropriate community care.

The post is open to internal applicants only and offers a permanent contract (full- or part-time) on Agenda for Change Band 4, salary £27,485–£30,162 pro rata; 12-month secondments will be considered. Essential requirements include NVQ/Diploma Level 4 or Foundation Degree in Health/Social Care (or extensive relevant experience), the Care Certificate, GCSE-level English and Maths (or equivalent), prior hospital discharge experience and good IT skills.
