Job summary
We are seeking a Frailty Nurse to work 22.5 hours per week across Monday, Tuesday and Wednesday as part of our well-established Assessment and Care Team (AC Team).
Our AC Team support frail and housebound patients registered at our four practices as well as residents in Care Homes.
Main duties of the job
To provide a proactive approach for the populations of the four Practices in the Central
Bournemouth Primary Care Network (PCN), deemed to be at risk of admission, with the aim
of providing the support to remain independent at home and to reduce unnecessary hospital
admissions.
About us
We are a Primary Care Network comprised of four GP surgeries with 50,000 patients.
Our PCN-wide teams and services are based at East Way Clinic which operates as our network hub.

The role is intended to be complementary to the existing general practice and community
services, and to address a perceived gap in the support of people who may not have
immediate nursing, medical and therapeutic needs but would benefit from regular
assessment. The purpose of which would be to screen for common age-related conditions,
provide advice on accessing preventative services and build confidence with self-management.
