Main duties of the job
The post holder will work under the supervision of theclinical team and undertake appropriate delegated duties. They will workcollaboratively with the team to meet the needs of patients, following policyand procedures to deliver high quality responsive care.The primary responsibility of the HCA Navigator will be toprovide an initial assessment of patients attending the service including,confirmation of identity and presenting complaint, determination of appropriateclinical stream and priority. If navigated to Mastercall PCC, vital signs willbe recorded including, temperature, pulse, blood pressure, respirations, oxygensaturation, blood glucose and urinalysis as required. Patient clinical prioritywill be established utilising a set of tools validated for use within theservice. The post holder will liaise with the senior clinician on duty tofacilitate urgent clinical assessment as required.The role is based within Trafford Primary Care Centre atTrafford General Hospital and the HCA will work as a member of the clinicalteam, supporting registered healthcare professionals in the delivery of anunscheduled primary care service delivered by Mastercall Healthcare.
