Job description
As a Health and Wellbeing Coach with Newport & Central Primary Care Network in Telford (Shawbirch Medical Centre, TF5 0LW), you will lead a new, non-clinical service funded through Sport England Place Partnerships to help patients take control of their health through group clinics and community activity. The role combines face-to-face delivery in neighbourhood venues with phone and email contact, and gives you real influence to design session content, choose venues and target patients using data-led approaches such as Core20Plus5. You will work closely with social prescribers, care coordinators, pharmacists and the Active Partnership to connect people with peer support, local activities and self‑management tools, using coaching, motivational interviewing and behaviour‑change techniques. This fixed-term (12 months) full-time role offers flexible working including daytime and evening shifts, a salary of £34,000–£36,167 and the option of secondment; an enhanced DBS check is required and GCSEs in English and maths are essential, with NVQ Level 2 in Health and Social Care desirable.
