We have an exciting opportunity for a Health Navigator to join our Primary Care team at HMP Isle of Wight, a category B male training prison. Located on the Isle of Wight, 8 minutes from St Marys Hospital and 26 minutes away from Cowes Ferry Terminal.
In this role you'll be working with a multi-disciplinary team that will support you and help you succeed.
This position is full time, youll receive an annual salary up to £35,644 per annum - depending on experience.
Hours:37 hours- 8 hour shift patterns
8am-4pm
9am-5pmHow will we support you?

Candidates are required to have a Legal right to work in the UK and due to the nature of the role, offers of employment will be subject to additional vetting and security checks.
As a Health Navigator your responsibilities will vary. You will
- Coordinate care between HMPPS, healthcare, and social care providers to improve joint working, patient access, and continuity care
- Support individuals during early custody, including risk identification, induction support, and reducing vulnerabilities such as self harm
- Promote health engagement by increasing awareness of services, improving attendance, and supporting initiatives like Health Champions and social prescribing.
- Record and manage date using clinical systems, contribute to case meetings, referrals, forums, and maintain accurate confidential records.
- Maintain safety and compliance with prison security procedures, contribute to risk assessments, and respond appropriately to incidents.
