Job description
Work as a Healthcare Assistant for Shropshire Community Health NHS Trust at Stirchley Medical Practice (Stirchley, Telford TF3), visiting patients in their own homes to provide practical clinical care under the supervision of the community team. The role involves organising and delivering skilled clinical support, managing clinical risk within a delegated caseload, liaising with colleagues and external partners to ensure joined‑up care, and taking direction from senior staff when needed. You will be expected to complete the Trust’s in‑house development programme to gain extended skills such as insulin administration, injections and wound care; candidates should hold NVQ Level 3 in care, be competent in phlebotomy and wound dressings, have recent care experience and be able to meet the travel requirements of the post. This position is subject to a Disclosure and Barring Service (DBS) check.
