The post holder will provide Support and Time to an allocated group of service users and promote their Recovery. They will assist the Multi-disciplinary team to assess, plan, implement and evaluate individual care plans. The purpose of this post is to work as part of a team focusing on the direct needs of service users working across boundaries of care and organisations to promote social inclusion and promote active participation in an individualised recovery plan using the agreed set of Team values and skills to underpin their day-to-day work. Service users will have individualised care plans. Please note - This role is not eligible for sponsorship unless you meet criteria by temporary exemption from current changes to immigration rules put in force on 22/07/2025. Separate provisions are applied to workers who have been sponsored and held continuously Skilled Worker visa since prior to 04/04/2024. To demonstrate high–level communication To utilise highly developed interpersonal skills including specific skills to facilitate communication when there are barriers to understanding e.g. sensory or cognitive impairment or in a hostile, antagonistic or highly emotive situation. To establish and maintain robust communication networks with clients, carers, and other health care workers as well as external agencies. To be an active member of the multi-disciplinary team (MDT) To encourage and support clients to collaborate in their treatment programme and recovery. To encourage self-help as much as possible, promoting the maintenance of skills and independence in clients To support health promotion and act in a consultative role/advisor to other community services/agencies and personnel in relation to individual clients and their mental health issues.
