Job summary

The SEOxHA PCN have set up an Urgent Care hub at The Leys Health Centre.

We are looking for Paramedics and ANP's who are looking for part time work around their current role/an exciting full time role.

This hub provides excellent opportunities to engage in a primary care setting.

Main duties of the job

The three PCN practices will be able to book in to your acute hub clinic for same-day patients. Using your clinical expertise, the primary focus of the role is to work asan autonomous practitioner providing high-quality and timely patient-centred care.

The post holder will be responsible for theassessment, diagnosis, treatment, referral and discharge of a diverse range ofpatients with minor injuries/minor illnesses, often without reference to adoctor and within their level of competence.

Act as a qualified care practitioner, demonstrating advanced clinical competence and a knowledge base beyond those associated with conventional nursing roles.

Triage patients on our three practices Duty Doctor list and attend to patients either in a Minor Illness Clinic or remote consultation.

The post holder will work as part of a multi-disciplinary team in a primary care setting.

Undertake consultations with patients as an autonomous practitioner and using own clinical judgement to diagnose, treat, refer and/or discharge patients.

Make direct referrals to primary, secondary and social services within locally agreed pathways, guidance and protocols.

Ensure that patients receive high quality clinical care, delivered in a timely manner.

Ensure complete and accurate documentation of each and every patient contact.

Work as an autonomous practitioner and as part of multi-disciplinary and multiagency teams in order to ensure patients needs are met.
