We have an exciting opportunity for a self-motivated, experienced Occupational Therapist to join our A&E Therapy Team in Oldham Community.

We need people who share our excitement, embrace different ways of working and are able to work flexibly across organisational boundaries supporting the NHS Long Term Plan and admission avoidance agenda.

The post holder will work as part of the multi-disciplinary team, driving the changes to community care response to support the hospital avoidance agenda. The post holder will hold a rotational contract working across the Enablement and Urgent Care services, including Urgent Care Response and Intermediate Care bed bases. They will work in line with the agreed strategic objectives of Northern Care Alliance.

The aim of the team is to reduce unnecessary hospital admissions following attendance to the Emergency Department, by providing holistic therapy intervention and a person centred approach support in order to enable patients to return to their own home or usual residence. The team works closely with community services to support the home first model.

The successful candidate will be expected to work independently, but also as an integral member of the multidisciplinary team within the Emergency Department. The candidate will be able to demonstrate strategic planning and have a strong clinical background in Urgent Care or a similar field. Excellent time management and advanced communication skills are required along with awareness and understanding of local, regional and national agendas in respect to Health & Social care. You must be hold a UK driving license and have access to a vehicle for work purposes.
