Job summaryThepost holder is an experienced Nurse with a minimum of two years experienceworking as an autonomous practitioner. They should be working within theirprofessional boundaries and will provide care for the patients within both theirown homes and care homes as part of an integrated team within practices.
Theywill be skilled in initial history taking, clinical assessment, diagnosis,treatment and evaluation of care; whilst demonstrating a safe, criticalthinking clinical decision-making process and expert care delivery for thosepatients who are vulnerable and at risk of avoidable admissions within the generalpractice population.
Thepost holder will be integrated into practice teams, whilst working collaborativelywith the general practice team members, care home staff, patients, and otherHealthcare professionals to meet the needs of this group of vulnerable patients,whilst supporting the delivery of best practice, policy and procedures.
In order to work at this level NMC requirementsfor advanced practice must be met.
