DUTIES AND KEY RESPONSIBILITIES


Main duties of the job

Work within defined scope of practice to deliver high quality, compassionate care whilst undertaking specific clinical care tasks under the supervision of the lead practice nurse.

Deliver opportunistic health promotion using opportunities such as NHS Health Checks.

Discussing and sharing information with registered nurses about patients health conditions, activities and responses.

Work with supervisor to take responsibility for developing own clinical competence, leadership and reflective practice skills within the workplace.

Wound care management.

Administer medications as prescribed in accordance with NMC standards, e.g. via nebuliser or injections using sub-cutaneous or intramuscular route, under patient specific directions.

Delivery of the childhood and routine immunisation schedule, under patient specific directions or national protocols.

Monitoring and support in the management of patients with long term conditions / chronic conditions, using holistic patient centred approach.

Be actively involved in the provision of health education and self management advice to patients, relatives and carers.


Job responsibilities

Demonstrate good understanding of principles of consent and ensure valid consent is obtained prior to undertaking nursing and care procedures.

Provide care to people both in surgery and within their own homes.

Develop the skills and knowledge with supervisor to provide direct care to patients and families which may include (after additional training, dependant on surgery needs): venepuncture, vaccinations, wound care, contraception reviews, ear irrigation, spirometry and cervical screening.

Discussing and sharing information with registered nurses about patients health conditions, activities and responses.

Give smoking cessation advice and refer onwards to appropriate services for smoking cessation therapy

Undertake appropriate investigation, clinical observations and interventions following agreed guidelines for patients with chronic diseases such as coronary heart disease, hypertension and diabetes.

Meet the needs of patients presenting for wound care, using the local dressing formulary and guidelines.

Take venous blood samples from patients in line with practice protocols and infection control policy

Promote health and prevent ill health by understanding the evidence base for immunisation, vaccination and herd immunity

Recognise issues relating to safeguarding vulnerable children and adults and raise concerns to the appropriate registered care professional

a) The Practice operates a no-smoking policy, in line with Government legislation. This applies to all staff, visitors and patients. It is a condition of employment for staff that they do not smoke whilst on duty or in uniform or anywhere on the premises. This includes electronic cigarettes.

Infection Prevention and Control

a) Infection Prevention and Control is everybody's responsibility. All staff, both clinical and non-clinical are required to adhere to the Practices Infection Prevention and Control policy and make every effort to maintain high standards of infection prevention and control at all times thereby reducing the burden of Health Care Associated Infections.

Full Time Practice Nurse, Manor Park Surgery, Bramley, LS13 2UP, working Monday - friday between the hours of 07:30 - 18:00.

An exciting opportunity has arisen to join our nursing team as a Practice Nurse at Manor Park Surgery. You will work under the direct supervision of a lead nurse and strictly in accordance to practice guidelines and protocols.

This role will be subject to you passing a 6-month probatinary period.
