Job overviewAre you an experienced and enthusiastic speech and language therapist looking to join our innovative service supporting adults in the community.What do we aim for?o effective personal communication for peopleo safe, comfortable, enjoyable, and dignified eating & drinkingYou will provide specialist speech and language therapy to people with a range of complex medical conditions /social needs and associatedproblems, completing visits in patient's homes and outpatient clinics.You will receive one to one clinical support, professional development, and regular supervision. Giving you the opportunity to further develop yourskills in a variety of clinical areas such as our weekly Videofluoroscopy clinic, dysphagia management, with access to internal and externalcourses and much more.You will be working alongside wider multi-disciplinary teams within community services which is made up of physiotherapy, community nursing,occupational therapy, and social workers.You will work with colleagues in the wider LCO and acute across the trust. You will also work with colleagues from a variety of other agencies.

You will engage in clinical triage of new referrals. Whilst using advanced clinical reasoning and interpretation skills to analyse complex clinical andnon-clinical information to prioritise clinical urgency. Utilising specific Speech and Language Therapy skills to facilitate and optimise the treatmentoutcomes of patients. You will complete assessments of swallowing and communication disorders as required. To contribute to patient diagnosesand to recommend the most appropriate SaLT interventions. Communicating sensitively and effectively with patients and / or carers to promoteunderstanding and ensure inclusion in decision -making wherever possible.You will take responsibility for your own caseload in line with your experience and competence. Ensuring you reflect and evaluate own practiceand participate in supervision with colleagues
