An opportunity has arisen for a Community Support Worker (Band 3) for a 12 months fixed term contract.

This is a part time post and the hours are made up as follows: 8am -7pm weekdays and 9-5 at weekends and bank holidays for the Dementia and Older Person's Mental Health Team based at Drayton House in Chichester, West Sussex.

We are looking to recruit an experienced, enthusiastic and motivated Community Support Worker, to work within our experienced and friendly team supporting people living with Dementia and those with severe and enduring mental health problems. The team provides specialised mental health care within a recovery focused model designed to enable and encourage services users to develop positive coping strategies to maximise their independence and potential.

Main duties of the job

The role involves supporting the community nurses in delivering excellent service to the patients under our care. The successful candidate will be undertaking clinical visits to patients and their carers out in the community, be able to work as part of the multi-disciplinary team, feeding back any concerns thus ensuring risks are identified, discussed and acted upon within a timely manner.

You will be assisting the community nurses to plan, implement, review and evaluate individual patients care plans thus ensuring the patients' needs are at the fore at all times. The successful candidate will be expected to deliver safe and compassionate care and be pro-active in their approach to promote social inclusion and positive occupation in accordance with our Trusts values and behaviours.
