Job summary

Working in the beautiful city of York, the successfulapplicant will provide a practice nursing service to patients, includingchronic disease management and other specialist areas, taking appropriateinitiative and working independently.

The post holder works as part of the clinical team to meet theneeds of patients, leading when appropriate.

Main duties of the job

Provide the full range of nursing services, includingvenepuncture, ECG measurement, blood pressure measurement, urinalysis,vaccinations and immunisations (children and adults including travel), babyclinic, injections, cervical smears, swabs, travel services, health checks,assessment of risk factors, lifestyle advice, weight management, smokingcessation, suture removal, wound care, family planning, etc.

Manage the care of patients with long term conditions. This mayinclude organisational responsibility for specific disease areas.

Assess, plan, implement and evaluate treatment plans for patientswith long term conditions (or at risk of developing one) that meet identifiedneeds and promote health and well-being.

Select appropriate tests for patients and interpret and act onresults where appropriate.

Educate and support patients as needed, including adopting healthylifestyles, appropriate self-care for minor illness and following care plans forlong term conditions.

Responsibilities for patient care


Assess the needs of patients, providing appropriate care/treatmentin conjunction with the GP, or independently, as appropriate.

Refer to colleagues or other agencies as appropriate.

Provide the full range of nursing services, includingvenepuncture, ECG measurement, blood pressure measurement, urinalysis,vaccinations and immunisations (children and adults including travel), babyclinic, injections, cervical smears, swabs, travel services, health checks,assessment of risk factors, lifestyle advice, weight management, smokingcessation, suture removal, wound care, family planning, etc.

Manage the care of patients with long term conditions. This mayinclude organisational responsibility for specific disease areas.

Assess, plan, implement and evaluate treatment plans for patientswith long term conditions (or at risk of developing one) that meet identifiedneeds and promote health and well-being.

Select appropriate tests for patients and interpret and act onresults where appropriate.
