Band 6: £39,205 to £54,931 a year per annum inclusive of HCAs  PERMANENT 
Whittington Health NHS Trust introduced the Colorectal Straight To Test (STT) pathway in 2, successfully delivering effective services for suspected colorectal cancer patient referrals.

To assist / lead with the development of the Straight to Test Service for the 2-week cancer targets (suspected cancer referrals) in collaboration with the Colorectal STT Nurse / Lead Colorectal Nurse and ensure the service is covered for annual leave.

To set up and run telephone assessment clinics for Target referrals.

To run nurse led clinics for Holistic Needs Assessments and Remote clinics for cancer surveillance patients.

To assist with the 5-year cancer surveillance remote follow up programme in collaboration with the lead colorectal nurse specialist/administrator and colorectal team and to maintain and keep an accurate database for these patients.

The post holder will provide specialist nursing care to patients diagnosed with colorectal cancer. Will see both inpatients and outpatients, acting as the Keyworker for these patients.

To act as a resource of expert advice, carry own case load and provide coordination of care and support to patients with colorectal malignancies and their family / carer from diagnosis and through all stages of their disease, treatment modalities and follow up care.

To see all newly diagnosed patients with colorectal cancer, and to fast track and manage their pathway with the team from diagnosis to treatment within national guidelines.

To provide physical, psychological, social, emotional, spiritual, and cultural care to patients and their families affected by colorectal cancer. To complete a Holistic Assessment for patients at key stages of their pathway.

To work autonomously and liaise closely with other professionals involved in the patients care journey and provide specialist knowledge, expertise, and education for staff.

To continue to develop patient information in collaboration with colorectal team and Macmillan Information Manager.

To provide support for a Band 4 assistant practitioner.

We have an exciting opportunity to join our colorectal team as a colorectal straight to test nurse. This is a band 6 or 7 role dependant on experience (please see job description for skills and experience required).

The primary aim of the role is to assess patient's who have been referred onto the colorectal rapid access pathway and ensure they receive the most appropriate investigation
as soon as possible.

This service was born out of evidence that patients urgently referred for suspected colorectal cancer do not need to attend an initial outpatients consultation. If the patient is properly assessed at the point of referral they can be booked 'Straight to Test' e.g. colonoscopy. This model reduces the waiting time for the patient to be diagnosed and treated and will reduce the number of times the patient will need to come to the hospital thereby improving the patient experience.

The post holder will work alongside the established colorectal STT nurse as part of the colorectal team to continue to assess patients referred on the suspected colorectal cancer pathway.

The role will include conducting telephone and face to face triage assessments, booking appropriate tests based on the patients' symptoms and fitness, providing appropriate patient information, and booking an outpatient appointment for those unsuitable for STT.
