Band 7: £46,148 to £52,809 a year pro rata per annum  PERMANENT 

To undertake the comprehensive assessment of patients with neurological conditions, living in the community of west Norfolk, whose needs are complex and may have multiple health/social problems. Using investigative and analytical skills to assess and to formulate individualised management, through mutually agreed goals and treatment plans, using clinical reasoning, and utilising a wide range of treatment skills and options to develop a specialised programme of rehabilitation.

To have clinical responsibility for a caseload of patients, and to organise caseload using management skills, effectively and efficiently prioritising according to risk category.

Act as a resource on issues of neurological Physiotherapy practice to all colleagues within the Community Neurology Service, and to the wider integrated teams.

To provide specialist advice, teaching and training to other health and social care members of the wider multidisciplinary team regarding specialist neurological physiotherapy.

To train and supervise assistant practitioner/therapy assistant staff on a daily basis, and students where relevant, on matters of specialist physiotherapy practice.

To be professionally and legally responsible and accountable for all aspects of your own work including management of patients within your care. To ensure high standards of clinical care for patients under your management, and support more junior staff to do likewise.

To be able to communicate effectively with patients and carers to maximise potential and ensure understanding of condition and rehabilitation process. There are many aspects of communication, which may be required with a variety of patients, e.g. persuasion, explanation, motivation, empathy and defusing sensitive and stressful situations. There will also be some barriers to effective communication, e.g. loss of hearing, speech and language deficit, confusion/dementia, pain and anxiety/fear.

To be able to communicate effectively with other health or social care professionals in order to facilitate a multidisciplinary approach to the management of patients with Neurological Conditions in the community.

To undertake appropriate training to allow working across the professional boundaries of basic OT practice, e.g. assessment and issue of appropriate Activities of Daily Living (ADL) equipment

To work closely with the Speech and Language Therapist to assess and review patients with swallowing difficulties.

Be aware of Health and Safety aspects of your work, and comply with any policies that may be required to improve safety within work area, including prompt reporting of accidents to Service Manager, and ensuring equipment use is safe.

To undertake the measurement and evaluation of work and current practices, through evidence based practice, audit and use of outcome measures, either individually, or with team members.

To comply with professional standards of practice in accordance with professional body.

To maintain accurate, comprehensive and up-to-date documentation in accordance with professional standards and legal requirements, and within departmental guidelines.

We have an exciting opportunity for a Physiotherapist to join our award winning Community Neurology team, based in King's Lynn.

Our multi-professional team provides a specialist service for individuals across West Norfolk with a variety of neurological conditions for example MS, PD, MND and TBI.

The role includes working across the whole patient pathway from diagnosis and new symptoms management, through the condition pathway home based therapies, case management, social re-integration, vocational rehabilitation and palliative care. The post holder will be supported to provide specialist neurological assessment, interpretation, intervention and education. This will include patient-centred goal planning and rehabilitation and liaison/referral to other support services, and family support where appropriate in conjunction with the rest of the team.

Assessment and intervention will predominantly be held in patients home or at patients' choice of venue, including the gym, and other sites in the community where appropriate.

Undertake a major clinical role in assessment, review and treatment to the adult population with the diagnosis of a Neurological Condition and problems related to this, living in the community within West Norfolk.

o To plan, deliver and co-ordinate Physiotherapy intervention for patients with Neurological Conditions across West Norfolk

o To have an input into service development, under the direction of the Team Leads/ Service Manager, across West Norfolk and in conjunction with the rest of the team in response to NICE guidance, local governance and government policy.
