You will support and assist the Board of Directors in the provision of patient care

You will be responsible for the provision of skilled nursing care to the population of Whitby. You will deliver care within the boundaries of your role, focusing on supporting patients to be healthy, monitoring long term conditions, health prevention and screening activities. You will work collaboratively to meet the needs of patients, ensuring the highest standards of care, supporting the delivery of policy and procedures

You must work at all times within your own competence and professional Code of the Nursing and Midwifery Council NMC.


Duties and Responsibilities

The duties and responsibilities to be undertaken may include any or all of the items in the following list. Duties may be varied from time to time under the direction of the Medical Director or Board of Directors, dependent on current and evolving workload:


Clinical responsibilities

  • Assess plan, develop, implement and evaluate programmes of care to improve health and well-being and meet the healthcare needs of individuals.
  • Perform routine nursing tasks

Chronic disease management including CHD, CHF, COPD, Asthma, CKD, Diabetes and hypertension

BP checks

Chaperone and assist patients

Contraceptive Services (including Pill checks, injectables & emergency contraception)

Height, weight, BMI

Injections any, including Prostap and Zoladex

Immunisations

MRSA swabs

Urinalysis

Blood tests for adults and children aged 8-16 years

Cervical screening

Vaginal swabs

Dressings simple and complex

Removal of sutures and clips

Ear care and syringing

ECG

HRT follow-up

Learning Disability Health check

  • Prepare specimens for investigation by the pathology laboratory
  • Provide a holistic approach and deliver evidence based care
  • Administer and monitor medication consistent with protocols, standards and legislation
  • Enable patients, families and carers to develop their knowledge and understanding about their health and well-being and be as self-managing as possible
  • Prioritise health problems and intervene appropriately to assist patients in complex, urgent or emergency situations, including initiation of effective emergency care
  • Assist in the provision of nursing care within the context of primary care for patients with mental health problems
  • Offer advice about adult and child vaccinations and administer vaccines according to current guidelines and legislation
  • Advise and administer vaccinations for patients travelling abroad according to current guidelines and legislation
  • Delegate aspects of care clearly and appropriately, adopting the principles of safe practice and assessment of competence
  • Ensure that all daily checks are recorded accurately and in a timely manner eg vaccine fridges, defibrillator, emergency equipment, oxygen
  • Assist the maintenance of stocks of vaccines and other drugs according to current guidelines and policy


Other responsibilities

  • Recognise and work within own competence and in accordance with the professional Code of the NMC
  • Support should be provided to other clinical team members as required

Monitor the safety and effectiveness of own clinical practice

  • Maintain accurate and complete documentation and records utilising IT as appropriate
  • Use own judgement, resourcefulness and common sense
  • Follow agreed protocols, referring to GPs as appropriate
  • Follow local and national Infection Control guidelines
  • Assist with the maintenance of equipment and report defective equipment
  • Take responsibility that any equipment either clinical or otherwise, used when working, is taken due care of and locked away at the end of every working session

Maintain stock levels of clinical equipment and supplies, reporting to the Improving Access General Manager as required
