We are seeking an enthusiastic, well-motivated team member to join our Discharge and Patient Flow team. As a Patient Flow Support Worker, you will play a crucial role in facilitating safe and effective patient discharges, gathering and inputting data, and ensuring smooth flow within the hospital.
Patient Flow
- Facilitate safe and efficient bed movements and discharges
- Support daily bed meetings, gather and input data
- Assist with system access, knowledge of local IT systems
- Contribute to maintaining accurate bed states across the organisation
Discharge
- Work alongside Integrated discharge service/Ward teams to ensure safe discharges are followed by the ward.
- Coordinate incoming/outgoing telephone calls and email communications between wards/departments.
- Record data accurately and liaise with external partners
Requirements
- Excellent communication skills
- Willingness to learn new skills
- Some understanding of patient flow and safe discharge
- Ability to follow instructions and work independently when required
