We are seeking an enthusiastic, well-motivated team member to join our Discharge and Patient Flow team. As a Patient Flow Support Worker, you will play a crucial role in facilitating safe and effective patient discharges, gathering and inputting data, and ensuring smooth flow within the hospital.


Patient Flow

• Facilitate safe and efficient bed movements and discharges
• Support daily bed meetings, gather and input data
• Assist with system access, knowledge of local IT systems
• Contribute to maintaining accurate bed states across the organisation
  
  

Discharge

• Work alongside Integrated discharge service/Ward teams to ensure safe discharges are followed by the ward.
• Coordinate incoming/outgoing telephone calls and email communications between wards/departments.
• Record data accurately and liaise with external partners
  
  

Requirements

• Excellent communication skills
• Willingness to learn new skills
• Some understanding of patient flow and safe discharge
• Ability to follow instructions and work independently when required
