Assessment of patients on reablement unit including mobility, functional assessments and cognitive assessments.

Assessing for and ordering of appropriate equipment.

Delegation of appropriate work to Therapy Practitioners.

Multidisciplinary working with physiotherapy and nursing staff.

Discharge planning including liaising with relatives and external agencies.

Completing timely referrals to appropriate teams to facilitate discharges.
