Summary of the Care Coordinator's Role
The role is based in our office at Letchworth Garden City, and includes travel across North Hertfordshire and Stevenage. The Care Coordinator's role reports to the Care Manager and responsible for ensuring all client visits are allocated to match suitable Care and Support Workers and to maintain appropriate office procedures.

Principal Responsibilities of a Care Coordinator

  • Answering incoming telephone enquiries.

  • Maintain communication with care and support workers and service users regarding rotas.

  • Building and maintaining relationships with other professionals we work alongside.

  • Identifying gaps in the rota and sourcing new packages of care via E-brokerage or privately

  • Processing new client referrals including arranging assessments and start dates.

  • Preparing appropriate documentation for Field Care Supervisors.

  • Permanent allocation and templating of Care & Support Workers to all client visits and ensuring all visits are always covered.

  • Ensuring all clients and Care & Support Worker documentation and electronic records are kept up to date at all times.

  • Ensuring all other records are kept up to date.

  • Assist with team meetings and event organisation.

  • Authorisation and management of annual leave.

  • Managing an area of care and support workers and service users.

  • To work closely with the Field Care Supervisor.

  • To ensure compliance with all office systems and procedures.

  • Production of reports and management information as required.

  • Being part of the on-call team which includes early morning, evenings and weekends.

  • Recording and addressing issues that arise ‘Out of Hours’.

  • Dealing with emergencies when On Call – which could include going to support the care assistant or person we support until emergency services arrive

  • Completing weekly manager reports for care manager

  • Keeping our CRM (Leads) up to date on People Planner

  • Work towards lead time of 3 weeks
