Job summary

During training, the post holder will be practicing autonomously within their agreed scope of practice, stage of training and level of competence to provide patient[1]centred quality care. This will encompass the skills of advanced assessment, examination, diagnosis and management.

The tACP educational programme will blend academic university study with the acquisition of work-based clinical learning. The successful candidate will be fully supported to undertake a Post Graduate Diploma/MSc in Advanced Clinical Practice.

After successful completion of agreed objectives, the post holder will progress to a band 8a ACP. The trainee Advanced Clinical Practitioner will undertake the requirements of the Centre for Advancing Practice's framework and guidelines for advanced level practice. To work within the Multi-professional framework for advanced clinical practice in England (HEE 2.

Main duties of the job

Autonomously work in partnership with individuals, families and carers, managing a caseload of patients using a range of advanced assessment methods as appropriate (e.g. history taking, physical examination, identifying risk factors, mental health assessments).

Use expertise and decision-making skills to inform clinical reasoning approaches when dealing with differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses.

Practice in compliance with their respective code of professional conduct.

Negotiate an individual scope of practice within legal, ethical, professional and organisational policies, governance and procedures, with a focus on managing risk and upholding safety.

To work within locally agreed advanced clinical practice scope of practice, competence and capability being responsible and accountable for their decisions, actions and omissions at this level of practice.
