The role of a Social Worker at Pennine Care NHS Foundation Trust in Ashton-Under-Lyne involves managing a caseload of service users as part of a multi-disciplinary team focused on supporting individuals in their recovery. The post holder will be part of a multi-disciplinary team providing recovery.

- To be responsible for a caseload of service users under the care of the Team, supporting them in their recovery journey.
- To provide specialist social work interventions to service users and their carers / families, and involve them in all planning, helping to develop social resilience, motivation and daily living skills
- To offer guidance and leadership to Team regarding social care legislation and policy
- To take the lead on new bio, psycho, social assessments and review.
Provide for the social care needs of service users.
The post holder will be part of a multi-disciplinary team providing recovery.
- Visit families for information gathering and to offer support and provide carers assessments where needed.
- To attend MDT ward meetings, reviews, admission and discharge care planning.
- To provide professional Social Work support and advice to ward staff and other professionals in the multi-disciplinary team
- Deliver psychotherapeutic interventions with other members of the team
- To communicate complex assessment information care and support planning and risk factors, to service users, carers, professionals, and the MDT as appropriate and in line with Information Governance.
- Communicate sensitive issues in a skilled and appropriate manner and address any barriers to participation and understanding.
- The post holder will be able to form therapeutic, meaningful relationships with patients and ensure they are actively involved in both the planning and delivery of their care within a secure environment.
