The Torbay and South Devon NHS Foundation Trust is seeking a Health and Social Care Coordinator for a 30-hour per week position in Torquay. The role, scheduled from Monday to Friday with some weekend and bank holiday shifts, involves serving as a central point of contact for community health and social care referrals. Responsibilities include conducting needs-led assessments, applying Care Act eligibility, and managing risks, while also providing advice and information to service users and connecting them with relevant local services.

Candidates should possess strong communication and IT skills as well as a solid understanding of Care Act legislation. The role requires prior experience in a health and social care environment, the ability to handle multiple tasks and prioritize effectively, alongside the autonomy to manage a caseload and collaborate with the multi-disciplinary team. The coordinator will also formulate support plans, manage hospital discharges, and ensure timely responses to community health and social care inquiries.
