The role of "Out of Hours Sister/Charge Nurse - District Nursing" at Manchester University NHS Foundation Trust involves providing high-quality specialist nursing care to patients in community settings during out-of-hours periods. The post holder will exercise clinical judgement and decision-making, maintain accurate documentation, and support the development of junior team members while managing their own patient caseload. They will also take on leadership responsibilities in the absence of the team leader, ensuring adherence to nursing policies and clinical standards.
The successful candidate will play a key role in assessing, implementing, and evaluating patient care, particularly for those with urgent and complex health needs. Out of Hours District Nursing
The post holder will work with patients, families, carers to provide high quality specialist nursing care for patients in the community settings during the out of hours.
He/she will exercise enhanced levels of judgement, discretion and decision making in clinical care and ensure all records and documentation are completed and relevant. He/she will demonstrate flexibility and adaptability to autonomously.
The post holder will support the team leader with innovative clinical management and leadership of the team, mentoring the continued professional development of individual team members and assisting in the continued development of the District Nursing Service as a whole. The post holder will have delegated responsibility for the supervision and day to day management of junior team members.
He/she will contribute to the corporate objective of the Trust and in the absence of the team leader will take responsibility for implementing agreed nursing policy and clinical standards, monitoring staff compliance of agreed criteria.
The post holder supports the Team Leader in the continued development of the District Nursing Service as a whole.
Working with our Out of Hours Team, the post holder will manage a caseload of patients and support staff within their Team. They must carry continuing responsibility for the assessment of care needs, the development, implementation and evaluation of programs of care.
The post holder will be responsible for implementing agreed nursing policies and clinical standards, monitoring the compliance of staff with the above in accordance with agreed criteria.
The post holder will assess, implement, plan and evaluate the delivery of direct patient care. Their role involves the effective assessment and management of patients with urgent care, rapid access needs, the case facilitation and care coordination of patients with complex health needs.
