Job Overview
A fantastic opportunity has arisen for an enthusiastic and motivated individual to join the pharmacy team in supporting the rapidly expanding Hospital at Home services across MFT.
Hospital at Home promotes harmonisation of working across the acute hospitals and the community services within MFT, utilising the skills and resources available to successfully manage the acute care of patients whilst they remain in their own homes. We are looking for a General Pharmaceutical Council (GPhC) registered pharmacy technician with substantial experience of working within primary or secondary care. You will work alongside the pharmacist as part of a multidisciplinary team to provide medicines support and positively contribute to patient care. You should be confident in the ability to work independently with excellent organisational and interpersonal skills.
The successful candidate will become an integral part of the well supported and award-winning medicines optimisation team in providing a patient facing pharmaceutical service, working closely with the wider pharmacy teams and other services across MFT.
Main duties of the job
The post holder is responsible for providing technical input as part of the medicines optimisation support to the MFT Hospital at Home (H@H) service. This is a city-wide service integrating multidisciplinary teams (MDT) across both the acute and community services within MFT proactively supporting people to remain in their own home. The pharmacy technicians will work with the MDT to identify medicines optimisation interventions, carry out domiciliary visits to patients to complete medicines reviews and facilitate timely medicines supplies. They must also communicate across primary care interfaces at the point of admission and discharge to support seamless medicines information on transfer of care. The role requires participation in arrangements that may involve flexible working, e.g. extended hours or 7 day service.
