This is a hybrid role that works across health and social care settings as part of a rapid response MDT Team to prevent Hospital admissions. This is a fantastic opportunity for a Social Worker who enjoys working autonomously across different systems to support residents urgently on a daily basis. Your intervention will be short-term to facilitate short term packages of care prior to assessing longer term needs.

About The Role
This is a hybrid role that works across health and social care settings as part of a rapid response MDT Team to prevent Hospital admissions. This is a fantastic opportunity for a Social Worker who enjoys working autonomously across different systems to support residents urgently on a daily basis. Your intervention will be short-term to facilitate short term packages of care prior to assessing longer term needs.
The Main Duties In This Role Would Include:
- To work autonomously as a social worker, as well as a full member of the multidisciplinary team, providing a social work service to patients and their families within the community. Within the team, complete Care Act assessments for those in receipt of rapid response care/Hospital at Home to identify a safe, ongoing support plan.
- Complete Mental Capacity Assessments where appropriate and undertake Safeguarding enquiries to ensure residents receive support under Section 42 of the Care Act.
- To manage referrals from colleagues in the team within an agreed timeframe and to update the MDT regarding client work at required intervals. To consider diversity within the assessment process.
- To identify and assess risks to vulnerable residents and families and to work with other agencies and professionals to manage risk appropriately.
- To advise and assist residents and families in accessing appropriate community care services in conjunction with statutory and voluntary organisations.
- To provide information and advocacy about benefits and financial assistance, referring on for specialist advice as necessary.
- To coordinate and attend meetings, reviews and training courses as required.
- To use available information technology systems to prepare written reports, maintain case records in accordance with GDPR, local policy and professional standards.
- To attend virtual board roads and to track residents through the Hospital at Home process.
- To provide support and advice to colleagues within the team in order to better manage the residents’ social care needs.
- To work proactively to ensure integrations of services.
- To engage with professional supervision with regard to caseload and professional development.
- To work at all times within the appropriate legal framework, local policies and professional standards.
- The Hospital Social Work Team is a fast-paced person-centred service that places the needs of our residents first. We have a strong team work ethic. - Within your role as a rapid response social worker you will be community based and part of the MDT with the rapid response team.
- You will have contributed to a positive team culture and be a strong team member, with an emphasis on achieving collective goals and supporting colleagues on a daily basis.
- You will bring experience of delivering positive outcomes for residents that you have worked with and demonstrate examples of where you have upheld human rights.
- You will have sound and detailed knowledge of legislation such as the Care Act 2, Mental Capacity Act 2005. You will require a good knowledge of Ordinary Residence statutory guidance and Safeguarding Adults procedures to ensure that you meet the needs of the residents we work with on a daily basis.
- You’ll have to demonstrate your ability to work to tight timescales in a fast-paced environment and be able to flexibly manage your day-to-day priorities.
- Good IT skills will be required.
- Assertive communication skills will be required as you will need to undertake the role of a Social Worker in an MDT setting which can be challenging due to the differing priorities of organisations working with residents.
