Primary Care Advanced Pharmacist

Full time (1WTE / 37 hpw)

Location: Pharmacy Team, Aberdeen City HSCP.

Aberdeen City Health and Social Care Partnership (HSCP) has an exciting opportunity for a pharmacist to join our team and play a key role in the ongoing provision and innovation of the pharmacotherapy service across Aberdeen City.

Working with pharmacist and technician colleagues and with support from the HSCP lead pharmacists and pharmacy team co-ordinators, the post holder will work across GP practice(s) and the pharmacy hub, covering all aspects of medicines management and pharmacotherapy to ensure safe, appropriate and cost-effective use of medicines in order to deliver high quality pharmaceutical care for patients. You will have a role in supervising, training and mentoring other members of the pharmacy team

As an Advanced Pharmacist, you will be expected to lead, oversee and develop the following within your allocated GP practices: medication/polypharmacy reviews; medicines reconciliation; actioning hospital discharge letters, actioning acute and repeat prescription requests; providing prescribing advice to any member of the health care team; answering ad-hoc queries; attending multi-disciplinary team meetings; and supporting social care colleagues with medication-related issues in care at home services..

This post would suit pharmacists who have extensive experience in any sector of pharmacy and wish to progress in an advanced role within primary care, both clinically and with additional responsibilities for supervision, mentoring and leadership.

Candidates must have a degree in Pharmacy and be on the practising register of the General Pharmaceutical Council (GPhC). Candidates must also be an Independent Prescriber (IP) and be willing to develop in this role. Completion of a clinical diploma/ Advanced Competency Framework (or equivalent knowledge demonstrable through experience) is also essential. Applicants should ensure they specify their qualifications / experience in their application form in order to meet the essential shortlisting criteria (detailed in the person specification) especially where the required knowledge is gained through experience rather than a formal qualification or competency framework.

Candidates should have good IT skills; excellent communication and interpersonal skills; be a car owner and driver; and be able to work in any location in Aberdeen city.
