Care Coordinator role
Band 3 - 25 Hours per week, Permanent
We are looking for a Ward Coordinator to join our team in patient flow. The successful candidate will have a determination and a keen ability to help our patients and staff to maintain the adequate flow of patients to enable our patient to be treated and discharged in a timely manner.
You will liaise with the medical and nursing teams to facilitate safe and appropriate patient discharges.
Working alongside the nursing team you will assist in patient referrals, assessments, and taking admissions from the emergency department and internal departments, you will also undertake minimal patient audits.
This role will involve working alongside IUCT and the discharge team to facilitate discharges and maintain patient safety.
