The Cardiff and Vale University Health Board is seeking to hire two Memory Link Workers for the Memory Assessment Service in Penarth, funded by the Welsh Government to enhance dementia care standards. The role involves providing post-diagnostic support to individuals and their carers living with dementia, serving as the primary contact from diagnosis through end-of-life care. Responsibilities include signposting to services, connecting with community resources, and promoting their services in areas with low dementia diagnosis rates, particularly in diverse populations. The role emphasises person-centered support through meaningful conversations and requires mapping services within local GP Clusters. Welsh language skills are desirable but not mandatory.
