Surrey Downs Health & Care
The Rehabilitation Support Worker role will function as part of the new and exciting Recovery At Home team by providing care, support and rehabilitation within a community setting to provide a seamless service and to support discharges from hospital or prevent admission to hospital. The post holder will undertake a range of delegated tasks, following a care plan and will report to the Team Lead. This role will be supported to develop skills and abilities to work with this client group to provide the appropriate level of care.
We are looking for dynamic and caring individuals who will play a key role in driving our Discharge to Assess services in Surrey Downs. You will have excellent organisational and time management skills and be passionate about delivering holistic care that has the patient at its centre. You will relish developing new ways of working to improve quality and develop a "one team" culture. Part time opportunities are available for applicant seeking Morning shifts.
Main duties of the job
Home First supports the approach of "why not home, why not today?" Wherever possible patients should be supported home for ongoing assessments where this is not possible alternative pathways are available. Staying in hospital for longer than necessary has a negative impact on outcomes and ensuring patients are given the chance to continue their lives at home is vital for their long-term wellbeing.
@home works across organisational boundaries and as an integral part of the integrated health and care team is responsible for providing rapid access, enhanced, short term health and care support for people who would otherwise need emergency hospital admission as well for as those medically stable to return home from an acute hospital but requiring enhanced support as one stage in their on-going care pathway. The @home service spans the acute and community providing joined up, co-ordinated care to local citizens with health and social care needs. The Recovery Athome Team will form an integral and essential part of Discharge to Assess, providing timely care and support to facilitate discharge from hospital and onward assessment and identification of need. The team will work closely with health and social care services
The service provides a 7 day / week service from 8am to 8pm and staff are required to work flexibly over the full week
It is anticipated that this role will continue to develop through the acquisition of further skills, knowledge and competencies supported by the team

- Deliver clinical and therapeutic care to patients as per care plan.
- Perform patient assessment (under supervision and after appropriate delegation from the registered practitioner) plan and delivery high standards of care.
- Recognise the need for referral to alternative professionals and follow this through appropriately.
- Provide concise handovers to team Lead and/or other members of the wider integrated health and social care team
- Perform the role of link worker, for example, tissue viability, infection control or manual handling and feedback to members of the team any updated information.
- Provide and promote health education specific to the clinical area and in line with national and local policies.
- Promote independence and assist and support (where necessary) patients / clients in the activities of daily living.
- Promote patients in maintaining their personal hygiene, grooming and dressing needs with specific concern for their religious, cultural and personal preference ensuring dignity and privacy at all times, assisting when necessary
- To be aware of physical, psychological, social, cultural and spiritual needs of the dying patient.
- Use risk assessment tools appropriately to identify and reduce risks to patients and staff to ensure safe practice e.g. moving and handling.
- Undertake and perform clinical skills and observations against identified competencies to enhance the delivery of patient care e.g. vital signs, urine testing, blood glucose monitoring, venepuncture, oral pharyngeal suctioning, removing clips, sutures and 12 lead ECG recording, PEG and tube feeds, medication and deliver personal care.
