We have an exciting opportunities for an individual to join our hepatology admin team as a Project Co-ordinator-Pathway Navigator to support the delivery of Hepatocellular Carcinoma (HCC) surveillance where people at high risk of liver cancer are monitored on liver surveillance pathways. The post is band 4, 12 months fixed term based at St James hospital.

The post holder will identify existing patients with cirrhosis who are currently on surveillance and new patients identified through the Community liver health checks project and ensure that all surveillance patients categorised with cirrhosis are aligned on the cirrhosis pathway.

The post is fulltime, part time hours would be considered for suitable candidates.

Main duties of the job

The aim of the Project Co-ordinator - Pathway Navigator is to support the delivery of Hepatocellular Carcinoma (HCC) surveillance where people at high risk of liver cancer are monitored on liver surveillance pathways. The role

Provides direct support for patients to maintain engagement in surveillance pathways ensuring they receive surveillance scans on schedule and to identify and proactively find resolutions to barriers to care.

Co-ordinates and provide administrative support to the clinical teams linked with the pathway

Collates and update patient information on the relevant databases in line with local policy, to ensure efficient pathway management.

Ensure all contact details for patients on the pathway are up to date

Where necessary the postholder may be required to signpost patients to peers or social care for support.

The post holder will be required to work without supervision and prioritise workload

Job Purpose/Summary

To provide administrative and clerical support to the hepatology Department in Abdominal, Medicine and Surgery (AMS) CSU

The Project Co-ordinator - Pathway Navigator will support the delivery of Hepatocellular Carcinoma (HCC) surveillance where people at high risk of liver cancer are monitored on liver surveillance pathways. He/she will also be responsible for inputting data onto the departmental database system and producing reports as required. The post holder will be expected to arrange and plan her/his own work on a day-to-day basis within the overall demands and priorities of the department.

PRINCIPAL DUTIES & AREAS OF RESPONSIBILITY

Communication is key in this role as individuals will need to liaise with admin staff, clinicians, other departments within the Trust. There will be patient contact face to face and over the phone, therefore we need an individual with excellent communication skills, someone who is professional and friendly when dealing with colleagues and patients. The individual must be efficient ensuring that all tasks dealt with, and any queries received are answered or sign posted to the correct department or person in a timely manner or are resolved effectively.

You will need to have a flexible approach to work, be highly organised, enjoy working individually as well as part of a larger team and have good keyboarding skills and the ability to use excel to an intermediate level as a lot of the work will be using excel and producing data reports. Knowledge of hospital patient administration system is desirable but not essential as training will be given.

Work with the radiology department book patient surveillance appointments

Following up patients who DNA appointments to determine the reason and where required link to peer support (if available) to enable to the patient to attend rescheduled appointments

Work in a multidisciplinary team supporting the liver pathways from primary care into secondary care.

Provide day to day support for project activities which include project planning, risk and issue management, project reports

Design mechanisms for the collection, analysis and communication of complex data/information sets.

Ability and experience to carry out administrative duties such as filing, photocopying, mail handling, diary management, arranging meetings and events, taking minutes and following up actions where required.
