As a member of our friendly and supportive team you will be working to support the delivery of multiple sclerosis service provision for patients within the community across Fareham, Gosport, Havant, Waterlooville, Petersfield and Hayling Island. The role also entails a high proportion of advice and training for patients alongside other Health and Social Care professionals within the region, acting as a source of specialist advice for local professionals.
A keen interest in multiple sclerosis (MS) supported by experience of working with patients with MS/neurological conditions, as well as an MS qualification or a willingness to undertake this in the future is essential. Experience of teaching in any capacity would be essential and some experience of working within the primary care setting is preferred. In return for your skills this post offers a real opportunity for further development in this field of work and as such, as an employer, we would provide regular supervision alongside a detailed, tailored training program (both internal and external) to ensure you have the necessary skills to support the service effectively.
To deliver a specialist service providing specialist case management and educational support for patients with Multiple Sclerosis (MS) under the supervision of the Multiple Sclerosis Specialist Nurse Lead (MSNS), coordinating care with partner agencies and key stakeholders with the aim of promoting well-being, education for patients and healthcare professionals in order to prevent hospital admission by maintaining safe and effective care in the community setting.
