We are looking for an enthusiastic highly motivated leader with a vision to develop and maintain high standards of practice across the organisation. The role requires the individual to have a passion for service improvement to drive the team forward.

The post holder must be a caring and a compassionate individual focused on providing excellent quality improvement, working within the values held by the Trust for patients with a complex need (learning disability, autism, dementia, delirium, and mental illness).

The Lead professional for Complex Needs will be an integral part of the Safeguarding and Complex Needs Team. The purpose of the job is to lead quality improvement projects related to Complex Needs by effective leadership, monitoring clinical effectiveness, and education.

The post holder will create an environment of continuous quality improvement and professional development to enable all staff to care for their patients with complex needs effectively.

Clinical

Develop a high level of clinical decision making, monitor and improve standards of care, working within their advanced scope of professional practice.

Contribute towards a multi-professional approach in the management of patients within the Complex Needs team, especially in relation to ongoing care needs and discharge arrangements.

Support patients and their families and ensure that they receive the required information to enable them to self-manage, lead, or participate in their care delivery.

To support, assist and encourage the multi-disciplinary team in resolving patient problems, by the provision of new and innovative models of case management.

Support the delivery of adjusted patient pathways for patients accessing secondary care.

Provide advice and support on the mental health act to ward staff and support the receipt and scrutiny of Mental Health Act Paperwork.

Leadership

Be an innovative, enthusiastic role model providing leadership, guidance, and advice to staff on operational and professional issues promoting an open and honest and transparent culture.

Provide professional visible leadership and a supportive approach that contributes to creating a climate for achieving excellence in clinical practice, skills and competencies using the values of the Trust.

In partnership with the Head of Complex Needs, lead the implementation of service improvements for people with Complex Needs and the evaluation/monitoring of care through audit, complaints monitoring, and incident monitoring and patient/carer feedback.

In partnership with the Head of Complex Needs, lead the Trusts Complex Needs Delivery Group providing clinical advice, expert knowledge, and practical support to the implementation of the Complex Needs Forward Plan.

Identify, develop, and sustain mechanisms to support patient involvement and feedback.

Be a resource for health care professionals by being visible on the wards, available, and accessible for support and advice in relation to the management of patients in the speciality.

Provide leadership to the team and working with the head of complex needs, develop new roles / ways of working (High Impact Changes) linked to directorate business plans.

Lead on consultations and develop strategies for delivering effective care within a changing environment that improves the quality of care and health outcomes in the Complex Needs team.

Management / Operational

Employ effective decision-making skills to address complex issues and use effective change management skills to implement these.

Take the lead in investigation of incidents / complaints when required, including detailed reports / letters to complainants.

Work in partnership with the Head of Complex Needs to manage the Complex Needs team towards achieving directorate, and Trust objectives.

Identify and develop proposals and business cases as required for future service innovation.

Manage and develop and influence the development of practice / services in clinical areas through the business planning process / service development and in conjunction with Matrons, Managers and Lead Clinicians.

Delegate work to match capabilities and workload of team members.

Manage own and other resources effectively to budget ensuring quality service delivery within budgetary constraints seeking to reduce budgetary costs by ongoing review.

Monitor sickness, absence, and competence issues in line with Trust Policy.

Manage recruitment and selection of staff in line with Trust Policy.

Advocate

Ensure that high standards of nursing care are given and maintained and act when standards are not being met.

Involve patients and carers in the planning and delivery of care.

Provide opportunity and support for patients/families to discuss advanced care planning.

Encourage staff to Value the carer/family contribution and encourage a partnership of care approach supporting the underlying principles of Johns Campaign.

Be the patients advocate and ambassador respecting patient confidentiality and privacy with respect for diverse cultural backgrounds and requirements.

Lead the development of a patient focussed education and information giving strategy within the speciality.

Recognise, prevent, and diffuse potential areas of conflict and take a key role in the management of the outcome.

Clinical Governance / Quality

Ensure compliance with policies, procedures and clinical guidelines for self and others.

Develop and influence policy making and clinical guidelines internally and externally, necessary to support the Complex Needs team.

Continually monitor standards of care and lead the improvement of care through benchmarking, Nursing Charter Audits and Clinical Audit.

Support and facilitate research ensure evidence-based practice in the Complex Needs team.

Disseminate good practice and knowledge by pursuing opportunities to present work at national conferences or via publication and encourage team members to do likewise.

Promote patient and public involvement activities in the Complex Needs team leading to service improvement.

Promote peoples equality, diversity, and rights.

Identify clinical issues and incidents within the Trust that reduce the quality of care within the Complex Needs team brining any issues to the attention of the Head of Complex Needs.

Take part in activities to counter these to ensure the delivery of safe and effective care.

Participates and takes the lead in clinical audit within the Complex Needs team supporting others to participate and ensuring agreed results are implemented.

Education / Training

Takes responsibility for own continuing professional development and performance, including identifying own development needs and maintain own portfolio in accordance with re-registration requirements.

Participates in the supervision and the teaching of pre and post registration students, new staff members and junior medical staff.

To lead and develop with the Head of Complex Needs on educational programmes in relation to the speciality.

Supports the ward / department sister in ensuring the clinical environment is conducive to effective learning.

Identifies training needs and develop training resources for nursing staff to enable role development and to meet the needs of the service.

Take responsibility for providing complex needs core competencies for untrained and trained staff.
