Hastings & Rother Healthcare is seeking a dynamic Extended Scope Nurse or Paramedic for the Enhanced Care Team, specializing in frailty and palliative care. This full or part-time role offers flexible working patterns and involves providing high-quality, patient-centered care. The ideal candidate will join a multi-disciplinary team and be responsible for home visits, face-to-face clinics, and telephone consultations for patients with various medical needs, including minor illnesses and chronic disease management.

Key responsibilities include conducting holistic assessments, making clinical decisions, prioritizing patient needs, and coordinating care for palliative and end-of-life patients. The role also involves care for frail and elderly individuals, especially those in care and nursing homes, with regular ward rounds and assessments for low-level mental health concerns. The candidate must demonstrate effective clinical skills and a commitment to enhancing patient care.
