The Essex Partnership University NHS Foundation Trust is seeking a Community Mental Health Practitioner to join the new Mid and South Essex Older Adult Admission Avoidance & Home Treatment Team, based in Chelmsford. This part-time role offers 22.5 hours per week and comes with a salary ranging from £35,392 to £42,618 per annum (pro rata).

In this position, the practitioner will support older adults experiencing frailty and functional mental health challenges, focusing on admission avoidance and providing home treatment through a "Home First" approach. The role involves collaboration with a new team, existing community services, and established dementia admission avoidance initiatives across the region.

As part of the transformation agenda a new service is being formed to provide support to Older Adults, with Frailty and Functional Mental Health needs.
