The Avon & Wiltshire Mental Health Partnership NHS Trust is seeking a Band 6 Specialist Practitioner to join the Complex Intervention and Treatment Team (CITT) at the Victoria Centre in Swindon. The role involves providing specialized mental health services to older individuals with complex needs, including those with functional illnesses and dementia. Key responsibilities include care coordination for a caseload of service users, conducting assessments, developing and reviewing care plans, and delivering therapeutic interventions. The successful candidate should possess experience in community mental health care, decision-making abilities, and proficiency in IT systems, along with a valid driver's license.
